Git history - how to make it clean

The problem

We often struggle with messy git history. You've joined a new project, you'd like to glance through git history and you see many merge commits, unstructured commit messages, and branch names pattern. Commit messages sometimes start with a ticket number, sometimes with a type of commit and sometimes it's only a few words of description. The same case with branches naming pattern, but additionally there is an underscore vs hyphen case.
